Bile‐duct ligation renders the brain susceptible to hypotension‐induced neuronal degeneration: Implications of ammonia

Hepatic encephalopathy (HE) is a debilitating neurological complication of cirrhosis. By definition, HE considered reversible disorder, and therefore should resolve following liver transplantation (LT). However, persisting complications are observed in as many 47% LT recipients. an invasive surgical procedure accompanied by various perioperative factors such blood loss hypotension which could influence outcomes post-LT. We hypothesize that minimal (MHE) renders the brain frail susceptible to hypotension-induced neuronal cell death. Six-week bile duct-ligated (BDL) rats with MHE respective SHAM-controls were used. Several degrees (mean arterial pressure 30, 60 90 mm Hg) induced via withdrawal from femoral artery maintained for 120 min. Brains collected count apoptotic analysis. In separate group, BDL treated ammonia-lowering strategy ornithine phenylacetate (OP; MNK-6105), administered orally (1 g/kg) 3 weeks before induction hypotension. Hypotension 30 Hg (not significantly decreased marker expression (NeuN) cresyl violet staining frontal cortex compared hypotensive SHAM-operated controls well non-hypotensive rats. Neuronal degeneration was associated increase cleaved caspase-3, suggesting mechanism death apoptotic. OP treatment attenuated hyperammonaemia, improved anxiety activity, protected against Our findings demonstrate chronic disease more degeneration. This highlights at time risk factor poor outcome post-transplant treating pre-LT might reduce this risk.
